The Hospice Plan Of Care
One of the tools used in hospice that helps us provide highly individualized care to each patient, is the Hospice Plan of Care. Patients and family, together with the hospice interdisciplinary team, work to develop a structured treatment plan to best serve you or your loved one. This plan is unique to each patient, and is continually reviewed and adjusted, based on the patients’ needs and desires. Providing care and services that are responsive to the needs of each patient, and their family, is of the utmost importance to us. Both the patient and family become one in the hospice plan of care with CompassionCare. Advance care plans are an essential part of the hospice process, and treatments are chosen based on the patients’ wishes, beliefs and values, and can be adjusted as a patient’s condition changes.
Our individualized plan of care is holistic and takes into consideration our patients overall health condition, level of pain (if any), and other symptoms they may experience, such as anxiety or distress. We also consider many other conditions relative to each patient as a whole person, such as their emotional and spiritual needs. We truly understand our patients are unique human beings, not their medical diagnosis, and all people are not the same. The hospice care plan is designed and implemented with each individual’s needs as the primary objective.

Advanced care planning consists of making decisions prior to needing them. We understand that some of these decisions can be difficult to discuss. Under the hospice care plan, hospice staff visit on a regular basis, based on the patient’s needs. Patient receive medical care in the comfort of their own home. They are checked on, medications adjusted as needed, and support given to the patient, their family and loved ones. We have support services available not only to the patient, but to the family and caregivers, too. CompassionCare offers spiritual support, private one-on-one counseling, and bereavement services as well.
